2022-2023 Lamp Hard Copy Request Form

Form for submitting if you would like to receive a mailed copy of the Lamp. By default, the Lamp is emailed to the email address on the chapter roster provided to the Indiana State Council. Only use this form if you have no way to receive a copy of the Lamp besides being mailed one by the Lamp Editor. This form must be re-submitted each ESA year.
